Referral Information

We welcome referrals from waiver Case Managers, Social Workers, and/or Care Coordinators. In cases where an individual lacks a waiver, the referrer or Better Life Housing can arrange for the completion of a Professional Statement of Need (PSN). Additionally, the completion of a Housing Focused Person-Centered Plan (HFPCP) by targeted case managers or care coordinators is necessary. If not completed, Better Life Housing can either offer Consultation services to finalize the HFPCP or provide Transitioning/Sustaining Services. Better Life Housing can also assist referrers in finding an alternative HSS agency for Consultation or Transitioning/Sustaining services.

The Process

Once the referral process is initiated, Better Life Housing assumes responsibility for enrolling the individual with the Department of Human Services (DHS). Upon approval, a designated member of the Housing Stabilization team will schedule an intake session with the individual to better understand and address their needs. It's important to note that HSS primarily caters to individuals aged 18 or older, with a disability, and active Medical Assistance Coverage.

For HSS Clients seeking to change providers, services will not commence until DHS approves the Provider Change, which typically takes 3-4 weeks. Conversely, new enrollments into HSS can usually begin services almost immediately, barring any special circumstances.

How To Fill Out a Referral

Individuals lacking waiver, targeted case management, or an MSHO/MSC+ care coordinator, yet requiring Housing Stabilization Services, can fulfill the necessary person-centered planning through an enlisted housing consultant. These consultants aid individuals in crafting a Housing Focused Person-Centered Plan (DHS-7307) and guide them in choosing their housing transition or sustaining provider. We can assist in locating a housing consultant to fulfill this aspect and enroll your client in Transitioning/Sustaining services.

Once Referrals Are Submitted
  • Once referral form and CSSP has been received, Better Life Housing will contact the Case Manager to better learn of the client’s case and set up a timeline for support

  • Better Life Housing will submit client for eligibility through DHS

  • Clients who are newly enrolled into HSS services will not start until we receive an approval letter from DHS.

  • Clients who are looking to submit a Provider Change Request will not begin services until DHS Approval Notice of provider change has been completed (3-4 weeks)
